Waiver Option
Maple Shade Schools

If eligible, you have the option to waive your District health benefits in return for a cash payment. This waiver option offers flexibility in creating your own personalized package of benefits. If you have health benefits elsewhere, you may be eligible; see below waiver form for details. Note: your waiver payment is considered taxable income.

If you lose coverage from the alternative source, you can immediately enroll in the District plans by completing an enrollment form and providing documentation of the loss; email the Business Office (cdudley@msemail.org / x5280) for instructions.
